Pharmacies are enforcing strict policies on filling/refilling of controlled/narcotic prescriptions. For example, if a patient fills a 30-day supply of oxycodone on June 1, the next due date would be July 1. Most pharmacies have policies to only fill these prescriptions a maximum of one or two days early.

Example #: Formulas Commonly Used To Create Insulin Dose Recommendations

This example illustrates a method for calculating of your background/basal and bolus doses and estimated daily insulin dose when you need full insulin replacement. Bear in mind, this may be too much insulin if you are newly diagnosed or still making a lot of insulin on your own. And it may be too little if you are very resistant to the action of insulin. Talk to your provider about the best insulin dose for you as this is a general formula and may not meet your individual needs.

The initial calculation of the basal/background and bolus doses requires estimating your total daily insulin dose:

How Many Insulin Vials

To quickly calculate the number of vials necessary for a 90-day supply of insulin, take the total daily dose and divide by 10. For example, if a patient is taking isophane insulin 30 units b.i.d., the TDD is 60 units per day 60 divided by 10 is six. Therefore, six vials are needed to cover the patients basal needs for the 90-day period. You’ll Need To Calculate Some Of Your Insulin Doses

You’ll also need to know some basic things about insulin. For example, 40-50% of the total daily insulin dose is to replace insulin overnight.

Your provider will prescribe an insulin dose regimen for you however, you still need to calculate some of your insulin doses. Your insulin dose regimen provides formulas that allow you to calculate how much bolus insulin to take at meals and snacks, or to correct high blood sugars.

Example #: Carbohydrate Coverage At A Meal

First, you have to calculate the carbohydrate coverage insulin dose using this formula:

CHO insulin dose = Total grams of CHO in the meal ÷ grams of CHO disposed by 1 unit of insulin .


For Example #1, assume:

  • You are going to eat 60 grams of carbohydrate for lunch
  • Your Insulin: CHO ratio is 1:10

To get the CHO insulin dose, plug the numbers into the formula:

CHO insulin dose =

  • The carbohydrate coverage dose is 6 units of rapid acting insulin.
  • The high blood sugar correction dose is 2 units of rapid acting insulin.

Now, add the two doses together to calculate your total meal dose.

Carbohydrate coverage dose + high sugar correction dose = 8 units total meal dose!

The total lunch insulin dose is 8 units of rapid acting insulin.

How To Calculate Insulin 7 Units And 24 Units

Regular insulin 7 units + NPH insulin 24 units is ordered for 7:30am. Indicate the dosage you would draw up first plus the combined dose on the syringe with arrows.     This example above assumes that you have a constant response to insulin throughout the day. In reality, individual insulin sensitivity varies. Someone who is resistant in the morning, but sensitive at mid-day, will need to adjust the insulin-to-carbohydrate ratio at different meal times. In such a case, the background insulin dose would still be approximately 20 units however, the breakfast insulin-to-carbohydrate ratio might be breakfast 1:8 grams, lunch 1:15 grams and dinner 1:12 grams.

    The insulin to carbohydrate ratio may vary during the day.

    The High Blood Sugar Correction Factor:

    Correction Factor = 1800 ÷Total Daily Insulin Dose = 1 unit of insulin will reduce the blood sugar so many mg/dl

    This can be calculated using the Rule of 1800.

    Example:


    = 1800 ÷ TDI = 1 unit insulin will drop reduce the blood sugar level by 45 mg/dl

    While the calculation is 1 unit will drop the blood sugar 45 mg/dl, to make it easier most people will round up or round down the number so the suggested correction factor may be 1 unit of rapid acting insulin will drop the blood sugar 40-50 mg/dl.

    Please keep in mind, the estimated insulin regimen is an initial best guess and the dose may need to be modified to keep your blood sugar on target.

Some pharmacies didnt have any consistent policies and procedures in place, leading to variation among pharmacists working the same store.

    Walgreens had designed their pharmacy dispensing system to prevent pharmacists from breaking boxes, forcing them to alter days supply to fit plan limits. They are accused of civil healthcare fraud and reach a $269.2 million settlement due to over-dispensing insulin pens.

    The FDA issues a Safety-Related Supplement Request to each insulin manufacturer requesting them to update Prescribing Information and carton labeling to specify that pens be dispensed in the original sealed carton.

    Manufacturers submit sNDAs and labels are updated . Now all labels state Dispense in the original sealed carton with the enclosed Instructions for Use.

    Semglee , a long-acting human insulin analogue, is approved in a single-pen carton size.


    The FDA clarifies the November 2019 update advising against breaking insulin boxes. They note an increased risk of dispensing errors and of patients using the wrong product if individual insulin pens are stored or dispensed outside of their carton.

    This app can be very helpful to determine the day-supply of medications. Really like that it includes drops and liquids. However, the reason I rated this app with 4 stars and not 5, it doesnt include a dedicated inhaler or allergy-spray day-supply feature. Inhalers and allergy sprays can require more effort to determine the correct day-supply. Taking the SIG and then having to determine how much medicine is contained within the product can take up more time. Luckily, most inhalers and other have meter-dosing or a certain number of puffs/sprays, so the calculations are more straight forward, but in the event that is not given a feature that could include that would be very helpful.

    First Some Basic Things To Know About Insulin:

    • Approximately 40-50% of the total daily insulin dose is to replace insulin overnight, when you are fasting and between meals. This is called background or basal insulin replacement. The basal or background insulin dose usually is constant from day to day.
    • The other 50-60% of the total daily insulin dose is for carbohydrate coverage and high blood sugar correction. This is called the bolus insulin replacement.

    Bolus Carbohydrate coverage

    The bolus dose for food coverage is prescribed as an insulin to carbohydrate ratio.The insulin to carbohydrate ratio represents how many grams of carbohydrate are covered or disposed of by 1 unit of insulin.

    Generally, one unit of rapid-acting insulin will dispose of 12-15 grams of carbohydrate. This range can vary from 4-30 grams or more of carbohydrate depending on an individuals sensitivity to insulin. Insulin sensitivity can vary according to the time of day, from person to person, and is affected by physical activity and stress.

    Bolus High blood sugar correction

    The bolus dose for high blood sugar correction is defined as how much one unit of rapid-acting insulin will drop the blood sugar.

    Generally, to correct a high blood sugar, one unit of insulin is needed to drop the blood glucose by 50 mg/dl. This drop in blood sugar can range from 15-100 mg/dl or more, depending on individual insulin sensitivities, and other circumstances.

    How Is Days Supply Calculated In Pharmacy

    How is Days supply calculated in pharmacy? Using a conversion factor of 20 drops per milliliter, calculate the days supply by dividing the total number of drops dispensed by the number of drops required for each days dose. For example, consider an ophthalmic product to be dosed one drop in each eye twice daily and dispensed in a 2.5 milliliter bottle.

    How many pills is a 30 day supply? Plans can have a day-to-day quantity limit. For example, your plan may cover one tablet of atorvastatin per day, so if you get a 30-day supply, youll only get 30 tablets of atorvastatin. Plans can also work in month time frames .

    What is days supply in Rx? In most cases, Days Supply is calculated by dividing the maximum amount of the medication used in 1 day into the dispensed amount. Days Supply means just what it sounds like: it is how many days the supply of dispensed medication will last.

    How do you calculate day supply for injections? To calculate the days supply for insulin, first calculate the total number of units to be dispensed by multiplying the number of units per milliliter by the number of milliliters to be dispensed. Then divide the total number of units to be dispensed by the number of units prescribed per day.

    Splitting insulin boxes has been an extremely confusing topic for community pharmacists. Before all this confusion started, insulin pens were dispensed based on how many pens would last for the corresponding days supply allowed by each patient’s plan limitation.

    When plan limitations were exceeded, pharmacies had to break up insulin boxes to provide the appropriate quantity or adjust the days supply to not break boxes. Pharmacies had different policies and procedures in place for how to handle this situation, leading to variation in practice.

    After Walgreens was accused of fraud for falsely understating the days supply and reached a $269.2 million settlement, retail pharmacies quickly rewrote their policies and started breaking boxes to dispense a more accurate quantity of insulin. By the time we thought the argument was settled, the FDA decided to jump in and stir the situation up even more by requesting not to break insulin boxes for safety purposes.
