With November being Diabetes Awareness Month, the occasion offers us an opportunity to be more aware of the medical advice we post to social media. Those who share information about these older insulins are inadvertently putting the onus on people with diabetes to stay well, even as such insulins are not effective for everyone. Diabetes advocates have repeatedly warned about the limitations of Walmart insulin but such messages continue to circulate unabated.
These memes regularly appear on my own social media, but they are never accompanied by a critique of insulin prices, the health care industry or the alleged price-gouging of pharmaceutical companies. In my analysis, these posts suggest that insulin is accessible, and if a person with diabetes does not pursue a cheaper option, it’s their fault, not the fault of opportunistic insulin manufacturers.
I believe Walmart insulin magnifies a class-based system of health care access, where people with insurance or money can access insulins that others cannot. Advocates like Laura Marston contend that the U.S. is the only developed country that pushes patients to utilize antiquated insulins rather than work to make analogs available.
The Problems With Walmarts Insulin
and diabetes advocates point out that while ReliOn may help patients in a pinch, especially those without health insurance, it’s also a formulation that came on the market in the 1980s, more than a decade before more refined insulins started to emerge.
The newer insulins, known as analogs, appear to be more effective at preventing dangerous blood sugar swings in people with Type 1 diabetes or those at a higher risk for severe low blood sugar.
There’s one more problem: Because it’s available without a prescription, patients can get the drug without the supervision of a doctor, and they sometimes get into trouble as a result. So stories have surfaced about patients who required emergency care because of severe blood sugar highs and lows after self-dosing with Walmart insulin, or even dying as a result.
“Human insulins are a reasonable option for many patients with Type 2 diabetes,” said Yale endocrinologist Kasia Lipska, who often prescribes the drugs for patients who can’t afford newer formulations. “But the drug isn’t optimal for everybody. And human insulin sold at Walmart is definitely not the solution to our insulin crisis.”
For people with Type 1 diabetes, human insulins “are harder to live on, lead to worse control, make it harder to hold down a job, impact quality of life,” said James Elliott, a trustee at , an independent patient advocacy group, “and not everyone lives near a Walmart.”

Walmart Inc. is debuting its more affordable, private-label insulin in its U.S. store pharmacies this week, the company said Tuesday. The diabetes medication will be in Sam’s Clubs nationwide by mid-July.
The Bentonville-based retailer said its ReliOn NovoLog is the first private-brand analog insulin. Manufactured by Novo Nordisk, it costs $72.88 per vial and $85.88 for FlexPens.
That saves customers between 58% and 75% off the cash price of brand insulin products, Walmart said, or up to $101 for brand vials and $251 for a package of brand FlexPens. The savings were calculated based on purchases without prescription drug insurance, the company said.
“The real innovation here is the price of the treatment,” said Dr. Cheryl Pegus, Walmart’s executive vice president of health and wellness. “We know many people with diabetes struggle to manage the financial burden of this condition, and we are focused on helping by providing affordable solutions.”

The term “generic” has been loosely applied to copycat versions of name brand insulins sold at lower prices, or to older, less reliable forms of human insulin. But the reality is there are no true generic insulins available — defined as chemically identical to brand-name products and with the same efficacy, but sold at much cheaper prices.
